I had an accident as well in December and the insurance company I dealt with didn't just give me money I had to provide them proof of everything. Did they agree in writing that they would pay you for pain and suffering along with missed time off?? If not then you won't have any recourse. They did pay your therapy and medical bills. You need to send in the estimate to the ins co. (make a copy) and let them know how much it is. You won't get paid for more than that. You should also send them proof of your car rental so they can reimburse you. Your back pain will be something that you will for the rest of your life, same as me. Might want to contact a lawyer about that and don't sign anything.
