I into Golds Gym to cancel mine and my wifes joint account back in March 2011.  At the time of our meeting with the Gold's Gym Representative, he acknowledged that we were both listed on one account.  In March 2011, our intention was to terminate both of our memberships altogether and we made this very clear to the Representative that was helping us.  He only had me fill out and sign a form as the primary account holder.  On April 26, 2011 the last Gold's withdrawal from our account occurred in the amount of $59.51.  After that, all debits stopped and we assumed our account was terminated.  Then, unexpectedly on 11/29/11, there was a debit for $194.67 on our account by Golds Gym South Central, a location we have never even visited. 

 I've spoken with that location off of Ben White several times over the past week and a half, but have yet to speak with anyone that can help me.  I do not understand how this $194.67 amount was authorized or why a 'South Central' Gold's would be involved.  In order to process this transaction, I was told they went into my wifes previous membership account to locate bank information.  This transaction should have never taken place.  If a debit had occurred the next month, we would have handled the situation immediately on only been out one months fees.  I have requested a refund and Golds Gym refuses stating it is my error despite the fact that their employee failed to present me with the proper forms at the time of my request.  How was I to know what I needed to fill out?
