Jack Koziol, president of Infosec Institute, has responded via email since my posting on this site with a very thoughtful and sincere apology. He has also graciously refunded the full amount I was charged for the class. In addition, Jack went beyond the refund to offer two additional classes of my choosing free of charge, including the hotel and food costs associated (I believe this is standard for II's training sessions). Jack has demonstrated that he is clearly determined at this time to make the situation right for myself and the others that were signed up for the training that went bad. Per Jack, the training failed to meet the expectations of Infosec Institute's standards due to the trainer himself. I believe at this time that Jack has every intention to fully correct the program and get it back on track to be beneficial for those seeking advanced ethical hacking training. He is emphatic that this is an isolated incident, solely on this course, and therefore will be easy for them to correct and get back on track.
