On April 28, 2014, I recieved an automated phone call from Netspend saying my card was locked and that I needed to provide the following information to unlock it, so I provided the card number and the expiration date.

The next day at 3:01pm it started and ended at 3:03pm. Each withdrawal was for exactly 303.00. Until there was less than $300.00 left in the account. My card was in my truck in my wallet.

These ATM withdrawals took place at PNCBANK 1100 S University Ann Arbor MI. I was in Phoenix AZ at the time, in the sleeperberth, I am an over the road trucker. I have electronic logs that prove this.

I call as soon as I found out, which was about 3 hrs later when I went to get some money out. I called Netspend and they said I needed a police report and an Unauthorized Use Affidavit with in 10 days, I asked where I can get this Affidavit and they repeated the same message. In otherwords I still do not know.

For the police report I called Phoenix Police Dept. and they said since the crime did not happen in Arizona they could not do anything. I have 4 more days to get this information together and continue to drive my truck. This is very difficult since I do not always have access to wifi.
