Was contacted by a insurer carrier of Perennial Care, they quoted me premiums that were much better than what I have been paying from a major carrier. They charged me a one time fee $99 as a fee extra to the premium. They told me no deductible, 100 percent coverage, ECT... Told me this was a great plan, the best out there. So, I was sold. It included separately eye, dental, rx at additional cost. But it was still $100 less than I was paying. I finally go to use it 8 months later and my payment for services at med express was denied. I'm now finding out it is a minor plan that if you have a cough your good but if you have anything major or surgery you are screwed. I've asked 3 times for packages sent to me that I can pick a primary care physician and see all my out of pocket cost and have yet to receive any thing. It's a rip off. Not worth the money, gives you bare minimum coverage. The government should investigate the company and it's practices and actually shut it down. Do not get this carrier. . EVER
