1. they must have sued you and won a money judgment against you. 2. They requested an enforcement order from the court to levy your bank account. Your bank will have a copy of this order from the court - get a copy of this document from your bank. This document will tell you what court ordered the levy. 3. Contact the clerk of the court that made the levy and awarded the money judgment against you to obtain copies of ALL DOCUMENTS filed in the matter, including PROOF OF SERVICE. 4. Contact a competant attorney to resolve this matter. You could try to fight it pro se, but I wouldn't recommend that - everything you file has to have all the i's dotted and t's crossed just so or else you loose. Good luck.
