Shoppers Advantage and Great Fun Tacked on to Allegiant Airline Ticket Connecticut

Around July 15, 2016, my aunt and uncle were killed in a car crash. I bought the ticket from Allegiant Airlines and got this pop up to save $50 on the ticket if I tried Shoppers Advantage for 30 days. I don't remember anything about the other company Great Fun. I was supposed to try it for 30 days and get to keep a gas card for $20 and a free gift card for $50. They said I would get something in the mail from them and the cards in the mail. I could cancel before the 30 days and keep the $20 and $50. They never sent me anything and I forgot about until I reviewed my bill to some unknown charges for TLG Shopper and TLG Great Fun. They have charged me $16.99 for Shopper and $16.99 for Great Fun since July. I canceled today. But don't know if they have already charged for September since they charged August 22, 2016. I told them I never received the $20 and $50 and they said I did. I did not! I have no addresses, no websites, just a phone number.
