It sounds like you're talking about a pre-existing condition information questionaire. This is actually something that is sent out to you and sometimes your primary care provider to make sure that you were not seen for a condition or if you had no break in coverage before becoming effective with an insurance company. Sometimes this is triggered by the receipt of a medical claim, probably the first one you submit to the insurance company. One thing to keep in mind is that this is not something insurance companies write into the plans, or actually enjoy doing, it is something that YOUR employer chose to have included in YOUR plan sp that they don't have to pay for an illness you may have before you started with them. If you do not agree with this practice you should really contact your HR Department and I'm sure they'll confirm this.
