Write them a letter, send it certified mail, return receipt requested, outlining what you wrote here, give names, dates and times of contacts, etc. Advise them that you believe this to be a case of identity theft and that by refusing to provide you with the appropriate paperwork, they are causing you undue harm. Cc that letter to the Attorney General's office in your state and the company's state, as well as your local District Attorney's office. You may also wish to contact your local DA and advise them that you believe you have been a victim of identity theft AND advise the credit bureaus.
