If you are just being assigned to an SMI clinic, DO NOT, under any circumstances go to Choices Network of Arcadia in Phoenix, unless you don't care about how you are treated. Choices has the worst reputation in the valley - even the taxi drivers know about Choices! The absolute WORST case manager is Jennifer Peralta. She will make you homeless, serve you with an Injunction, blame her mistakes on you and on others, and lies to cover her a*s. Her supervisor, Sahar Mohammed, is a big fat useless blob. Slow, ineffective and useless. Jennifer Lynn, another case manager, you will have to extricate information from. She will not willingly tell you anything. The two directors, Laurie Timerac and Letisha Hildebrand do not care and do not get involved. After signing a release for my records, they had 30 days to send them to me. Two months later I haven't received them. None of them care. Cold. Cold. Cold. All of them should be fired and barred from any position in the field of Mental Health. If you are SMI and are assigned to Choices, you will be SSSSSSSMI in no time at all. Hopefully Mercy Care will clean house...Magellan sucks.
