I was a patient a PEAK physical Therapy last year, I also am employeed with a local insurance company here in Colorado.
I just wanted to say in regards to insurance, unfortunatley it is ultimatley the patient' s resposibility to know their own benefits, some medical facilities check benefits as a courtesy, but also I have gone to doctor's offices before where they have no idea what the benefits are and ask me if I know what my co-pay is.
Also I am sure the facility has a contracted rate with your insurance company, which means that it doesn't matter if they bill the insurance $5.00 or $5,000.00 just for seeing you they get paid a flat rate, which looks like is $70.00 in your case.
on a final note, as I mentioned earlier, I was a patient of PEAK's last year for a little while and had a wonderful experience and would highly recommened the facility.

If I were you......I would make a call to your Insurance company's Fraud and Abuse department....or the SIU Unit and explain how this place is billing. They cannot bill you one thing and the Insurance something else.
