On November 22, 2004 I visited my doctor, the following day I submitted to Aflac a claim for coverage as allowed by my policy. As a new policyholder I patiently waited 4 weeks for payment of my claim. After 4 weeks I called Aflac only to find they had no record of my claim. While I could not speak with a "live" person the voice prompt system allowed me to request a set of claim forms.

When a set of claim forms arrived they were not suitable for the type of claim I am presenting. They required substantially more information than the first set. Also they required information far and above the demands of my claim. All this proved very time consuming and quite frankly a waste of my and my doctor's time.

I tracked down my regional representative, Allison Rothbar (410 979-1987) who said she would take care of my claim if I would fax her documented evidence of my doctor visit. This requirement, again, far in excess of the policy requirement and my original submission of claim some 6 weeks prior. Anyway, I followed her instructions, requesting from my doctor duplicate copies of my visit record and faxing them to Ms. Rothbar on January 13, 2005. Ms. Rothbar had promised an immediate payment.

I patiently waited until January 19, 2005 to receive payment on my claim. Having received no check or denial of claim I called Ms. Rothbar, who advised me that she had forwarded my claim for processing. First, I could have sent my claim directly and secondly, I was under the impression that a check would immediately be sent upon receipt of my fax. A fax which contained details far in excess of any policy/claim requirements. At any rate Ms. Rothbar said I should receive payment by mail. On January 19th I received, from Aflac Headquarters, another claim form identical to the one I submitted back on November 23, 2004. No check and no explanation!

On January 20, 2004 I call called the office of Mr. Daniel Amos (706 323-3431) President of Aflac who never returned my call. Later in the day I called Aflac Headquarters c/o their CFO who took some details and said he would have the claims department call me back. No one ever called.

Aflac's advertising and sales pitches make their coverage sound too good to be true! Well, if it sounds too good to be true, IT IS! Quack, quack.

All this over a $15.00 policy limited claim.

As of January 27, 2005 no check has been received.

If you are submitting a claim for a doctor's visit that pays $15 why dont you just fax in a copy of the doctor's visit (bill, etc) with your policy number and possibly a brief note of what you are filing for directly to the claims department fax number? FYI the number you should be calling is the 800 992 3522 # on the website. That's what customer service reps are there for. The 706 # will only get you transferred into the customer service dept. The commercials are not a sales pitch, they are truth. It's when the message is comprehended incorrectly that the confusion arises.
