Dear David, If I said I would help you then I must have requested you send me a copy of the UPC code because a lot of customers tend to send in an incorrect barcode (like the UPS label or the serial number) or sometimes it is not attached to the form and it gets lost. Please send me an email to [email protected] and I will instruct you on how to resubmit so that your rebate can be approved
