If you look on their website there are current addresses to send in a cancellation. Also my membership allows me to log-in to the website and there are printable forms for the training. Send it certified, always important if someone has your credit card number! Also don't forget the simple stuff, ask for help, ask for someone else who can assist you. If the staff doesn't know, they can't help you.
