My mother in law had burial insurance with Transamerica Life Insurance. My wife is the beneficiary of the policy, after contacting them the third time, she was told that they had no record of my wife ever contacting them. We were assured that the documents that needed to be completed were in the mail, and would also be e-mailed to us. Delay-Delay Delay!
